Responsibilities
- Deliver individualized occupational therapy plans for daily living and functional goals.
- Assess patients, document progress, and adjust treatment plans accordingly.
- Set measurable treatment targets and track outcomes against KPIs.
- Adhere to safety, compliance, and regulatory requirements in all activities.
- Collaborate with multidisciplinary team to coordinate patient care.
- Maintain accurate and timely clinical documentation in electronic health records.
- Educate patients and caregivers on therapy strategies and home programs.
- Demonstrate professional growth by pursuing ongoing training and certifications.
- Identify opportunities for process improvements to enhance patient outcomes.
- Participate in daily team huddles to align on patient flow and objectives.
- Engage in pipeline forecasting to anticipate demand and schedule capacity.
- Act as a mentor or peer resource to less experienced staff when appropriate.
Qualifications
- Hold a valid occupational therapy license and current registration.
- Deliver individualized OT plans with measurable daily living goals.
- Perform patient assessments, document progress, and adjust care plans.
- Ensure compliance with safety, regulatory, and accreditation standards.
- Track outcomes against KPIs and set target milestones.
- Collaborate with multidisciplinary team to coordinate care.
- Maintain accurate EHR documentation and timely data entry.
- Educate patients and caregivers on therapy strategies and home programs.
- Pursue ongoing training, certifications, and professional development.
- Identify process improvements to enhance patient outcomes and throughput.
- Engage in pipeline forecasting to anticipate demand and capacity needs.
- Participate in daily huddles and align on patient flow and objectives.
- Mentor or provide peer support to less experienced staff when appropriate.
- Demonstrate a growth mindset aligned with a target-driven culture.
