Occupational Therapist

Seeking licensed Occupational Therapists to provide personalized, in-home care to older adults in private residences and senior living communities. Responsibilities include conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy to enhance mobility, independence, and quality of life, and documenting care within EMR systems. Collaborate with an interdisciplinary team of PTs, SLPs, and Fitness Specialists, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Therapists may incorporate telehealth and virtual care to increase flexibility and patient support.
