Your role involves evaluating home safety, assessing functional needs, and implementing tailored treatment to promote independence.

Responsibilities

• Evaluate home safety
• Absess functional needs of physically disabled patients
• Develop, plan, and implement individualized occupational therapy treatment procedures
• Demonstrate strong assessment, treatment planning, and implementation skills for in-home care
• Excellently communicate for patient education
• Commitment to promoting independence and safety
• Provides occupational therapy assessment, care planning, patient treatment and education according to professional practice guidelines
• Works as a collaborative team member to develop a coordinated care plan
• Completes all required documentation and billing requirements according to established local, state and federal law
• Stays current with evidenced based practice guidelines and serves as a professional resource to other team members

Requirements

• Occupational Therapist license in IABasic Life Support (BLS) from American Heart Association

Preferred Experience

• Field of Occupational Therapy working in a hospital environment
