Occupational Therapist, OT

Responsibilities

• Perform outpatient therapy evaluations in designated senior living communities or group homes.
• Develop a plan of care for patients based on current research-based approaches to care.
• Maximize functional independence for clients through rehabilitative care and skilled nursing.
• Collaborate with home health therapy partners to transition care from home health to outpatient services.
• Achieve client-centered goals by working closely with clients, families, and community caregivers.
• Educate clients, families, and caregivers on care plans to support functional independence.

Qualifications

• A current NJ Occupational Therapist license.
• Prior home care experience preferred but not required.
• Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the American Occupational Therapy Association (AOTA).
• One year of recent work experience as an Occupational Therapist.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills, especially with electronic medical records (EMR).

Additional Information

Base Pay: $70.00 - $75.00 per point for full time status, $75.00 - $80.00 per point for per diem status.
