Responsibilities

• Conduct thorough evaluations and develop evidence-based treatment plans
• Effectively communicate with patients and caregivers to involve them in goal setting and interventions
• Monitor progress, adjust plans as necessary, and determine discharge readiness
• Collaborate with other professionals to ensure coordinated care
• Emphasize ongoing professional development
• Participate actively in team meetings and complete mandatory tasks

Schedule
