Occupational Therapist Asst (COTA) PRN

About the role

Spring Creek Rehab & Healthcare Center is seeking a PRN Certified Occupational Therapist Assistant to join our therapy team. We are a skilled nursing and rehabilitation facility focused on providing compassionate, family-oriented short and long-term care. This position offers a warm and secure work environment with flexible scheduling and no daily travel.

Responsibilities

• Screen, examine, and evaluate residents including history, systems review, and application of appropriate tests and measures
• Interpret findings to establish a diagnosis, identify impairments, determine predicted level of improvement and time required to achieve it, and identify precautions
• Design a plan of care and develop appropriate treatment goals and methods in collaboration with residents and caregivers
• Implement the treatment plan and complete all related documentation required for services
• Consult with and make referrals to other disciplines and agencies to address the comprehensive needs of residents
• Provide exceptional customer service ensuring residents and families receive the highest quality of care in a caring and compassionate environment
• Maintain efficiency standards

Qualifications

• Must have a valid professional license by discipline for state
• Prefer at least one-year experience in long-term care
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Passionate about post-acute rehabilitation and the older adult population
• Ability to relate and interact positively, effectively, and appropriately with residents, family, professional, and facility staff
