About the role

Garden City Health Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to join their dedicated rehabilitation team. The ideal candidate will have either experience or be a recent graduate who can assist the Occupational Therapist in providing direct patient care.

Responsibilities

  • Treat residents as directed by the supervising Occupational Therapist
  • Implement treatment plans according to physician orders
  • Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility policy, and payer requirements
  • Communicate resident progress, problems, and plans with the supervising OT and interdisciplinary team
  • Absorb training of Restorative Aides as appropriate
  • Participate in in-service training programs and care conferences
  • Educate residents, families, and caregivers on maintenance programs and discharge planning
  • Participate in discharge planning and transition of care
  • Maintain a clean, safe, and organized treatment area
  • Report equipment issues to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, and staff

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program
  • Skilled nursing or geriatric experience preferred
  • Current, active California Occupational Therapy Assistant (COTA) license
  • Must meet all continuing education requirements
  • Maintain license in good standing with the State Board

Skills

  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
  • Strong communication, organization, and problem-solving skills
  • Ability to work independently and as part of a collaborative rehab team

Physical Demands

  • Frequent standing, walking, reaching, pushing, and pulling
  • Ability to exert up to 50 pounds frequently and 100 pounds occasionally
  • Occasional climbing, balancing, stooping, kneeling, crouching, or crawling
  • Visual abilities include close, distance, color, peripheral, and depth perception

Work Environment

  • Skilled nursing and rehabilitation environment
  • Noise level typically low to moderate
  • Reasonable accommodations available
