Key Responsibilities
- Conduct comprehensive nursing assessments of patients initiating or continuing MAT (e.g., buprenorphine, naltrexone).
- Monitor patients’ clinical response, side effects, and adherence to treatment through regular follow-ups.
- Provide patient education on MAT, withdrawal symptoms, harm reduction, and relapse prevention.
- Administer or oversee administration of medications as needed; coordinate with prescribers on treatment adjustments.
- Perform and interpret toxicology screening and coordinate care based on results.
- Serve as the primary care manager for OBAT patients, ensuring continuity of care across settings.
- Develop and maintain individualized care plans in collaboration with the care team.
- Carefully coordinate referrals to behavioral health, primary care, social services, housing, and recovery support services.
- Aid in insurance authorizations, prior authorizations, and medication access barriers.
- Collaborate with other Dimock Addiction levels of care including ATS, CSS and Askia.
- Work closely with physicians, nurse practitioners, recovery coaches, and counselors to ensure holistic care.
- Participate in interdisciplinary team meetings, case reviews, and performance improvement initiatives.
- Maintain accurate, timely documentation in the electronic health record (EHR) in compliance with regulations.
- Support program metrics, data collection, and reporting for regulatory or grant requirements.
- Contribute to development and implementation of policies, procedures, and clinical protocols related to OBAT services.
- Educate clinic staff and patients on addiction care practices and emerging trends in MAT.
- Aids in other projects such as Addiction related clinic grants.
- Collects data for the bureau of substance addiction services on new patients, quarterly and at discharge: submits this data as required.
Qualifications
- Licensure: Current and unrestricted Registered Nurse (RN) license in Massachusetts.
- Education: Associate or Bachelor of Science in Nursing (BSN preferred).
- Experience: Minimum 2 years of nursing experience; experience in addiction treatment, behavioral health, or community health strongly preferred.
- Familiarity with MAT protocols (e.g., buprenorphine, naltrexone) and trauma-informed care.
- Certifications: BLS required. CARN (Certified Addiction Registered Nurse) or similar credentials preferred but not required.
