Nursing Supervisor, RN- Hospice (CovenantCare)

Essential Responsibilities

• Receives case referrals.
• Reviews available patient information related to case, including home visits, to determine hospice/home care needs.
• Sets up appropriate hospice personnel to cases as needed.
• Conferences with Medical Director regarding any questions about an individual's eligibility for services.
• Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians.
• Discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services.
• Is always available during operating hours to assist clinicians as appropriate.
• Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.
• Aids clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
• Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
• Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel.
• Aids in the screening and interviewing process of new employees and makes recommendations for employment of individuals.
• Aids in the orientation of new employees.
• Aids Clinical Director and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs.
• Aids in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.

Education And Work Experience

• Required: High School Diploma or Equivalent.
• PREFERRED: Associate’s Degree.
• Certification: RN.
• Experience: 3+ Years.

Knowledge, Skills, And Ability

• Excellent observation, communication, attention to detail, time management and organizational skills.
• Understands principles of pain/symptom management and hospice philosophy and issues of death/dying.
• Knowledge of medical practices and terminology.

Physical Requirements

• Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
• Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
• Ability to operate various equipment base on department.
• The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms).
• Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
• Clarity Of Vision For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
• For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
• For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation

Pay Range: $91,959.00 - $130,753.50 per year

Reasonable Pay Estimate: A reasonable estimate of the pay range for this position is $91,959.00 - $130,753.50 per year. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
