Location: Mount Vernon, NY
Perfect for Travel Required: 50% (Counties: Westchester, Bronx, Rockland, and Putnam)

Responsibilities

  • Provide health teaching to individual program participants.
  • Conduct health assessments (Health Assessment Forms) and social-related health care screenings.
  • Coordinate and collaborate with medical providers.
  • Complete all required documentation.
  • In the event of a medical emergency, assess, take vital signs, and coordinate additional medical assistance as needed.
  • Conduct and document medical and nursing services as required by the Guidance Center of Westchester and Network policies and procedures.
  • Assess and schedule the need for other periodic health check-ins or annual health screen form evaluations.
  • Review Medical Records and ensure accurate data entry into the electronic record.
  • Act as a liaison concerning medical issues with consulting physicians, parents/family, Case Managers, Day Program, Emergency Room, Site Supervisor, and other Interdisciplinary Team Members.
  • Review required nursing in-services with counselors.
  • Advise the nursing supervisor about necessary changes in the medical status of Program Participants.
  • Facilitate monthly staff training with housing staff.
  • Perform additional nursing responsibilities as required by the Nursing Supervisor.

Requirements

  • Current RN license to practice in NYS.
  • Two years of previous nursing experience with mental health, substance abuse, chronic medical conditions, public, or community healthcare.
  • Experience with adults in Mental Health and/or a closely related field.
  • Valid CPR/BLS/AED certification.
  • Must have a reliable working vehicle and a valid clean driver’s license.

  • Self-starter with a high level of patience, empathy, and compassion.
  • Excellent observation skills with high attention to detail.
  • Strong organizational, problem-solving, and time management skills.
  • Ability to instruct people supported and staff in medical procedures and healthy lifestyle practices.
  • Understanding of healthcare state standards and procedures.
  • Previous clinical experience.
  • Strong computer skills.

Pay

$55.00 per hour

Schedule

35 hours per week. Flexibility required.
