Nurse - RN - Skin Integrity Nurse

Responsibilities

• Completes weekly head to toe skin assessments and documents in the EHR.
• Ensures appropriate treatments and/or interventions are in place for skin issues identified during the skin rounds.
• Collaborates with the Clinician or Wound NP to obtain new orders or discontinue orders as needed.
• Updates the ADON and DON if any new or worsened ulcers are identified.
• Updates the ADON and DON with any non-healing wounds.
• Demonstrates proper infection control practices.
• Affixes to and participates in weekly SIT meetings.
• Ensures residents’ responsible parties are updated on wound progress.
• Proactively develops positive employee relations and promotes teamwork, mutual respect, and effective communication.
• Performs other duties as requested by the DON or ADON.

Qualifications

• Graduate of an accredited school of nursing with a current RN license for the state where the facility is located.
• Understands the integumentary system and willing to continue learning.
• Utilizes excellent communications skills to relate to patients, families, and staff members.
• Constantly updates knowledge through ongoing education.
