Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966), Outer Cape Health has a long history of successful growth and expansion in pursuit of this mission. Responsibilities

• Assist in triaging patient telephone calls
• Conduct intake on walk-ins to identify need for primary care services versus escalation to a higher level of care
• Support providers with patient care visits, procedures, outreach, and same-day walk-in care
• Implement orders for EKGs, dressing changes, suture removal, ear lavages, splinting, injections, IV insertion, and IV infusions (Provincetown Health Center only)
• Make appropriate referrals required for patient care management
• Obtain required patient specimens and perform or request diagnostic laboratory tests
• Communicate diagnostic laboratory results to providers and prioritize communication of abnormal results to physicians and patients (if requested)
• Render treatments as ordered by providers (e.g., intravenous, pulse, SpO₂ monitoring)
• Conduct routine patient examinations according to OCHS Nursing Protocols/standing orders (e.g., blood pressure checks, allergy injections, minor wound care, uncomplicated suture removal)
• Clean, wrap, and sterilize instruments according to policy and procedure
• Perform phlebotomy tasks when needed at the direction of a clinician
• Participate in quality assurance/quality improvement activities and training seminars (e.g., OSHA)
• Seek appropriate and timely guidance from the Nurse Manager based on the level of concern
• Provide the best patient experience by addressing concerns, explaining procedures/testing/diagnoses thoroughly
• Maintain patient privacy and confidentiality in accordance with OCHS policies and HIPAA
• Maintain accurate and timely documentation in the Electronic Medical Record (EMR)
• Educate patients on proper use of medical equipment, prescribed regimens, and other nursing care regimens
• Refer patients to outside sources, programs, or support groups for additional education or referrals
• Promote and support teamwork by collaborating with providers, Medical Assistants, and Patient Service Representatives
• Adhere to National Patient Safety Goals applicable to ambulatory care centers (e.g., improving patient identification accuracy, medication safety, reducing care-associated infections, preventing wrong site/procedure/person errors)
• Travel between OCHS sites as needed when directed by nursing leadership
• Work in remote settings as needed and directed by nursing leadership
• Perform other related duties and projects to ensure the needs of the Health Center and its patients are met

Requirements

For Registered Nurses (RN):

• Associate of Science in Nursing from an accredited school of nursing by AACN (BSN preferred)
• Current Registered Nurse (RN) license from the Massachusetts State Board of Registration
• Basic Life Support (BLS) Certification
• 1 year of professional nursing experience (ED, urgent care, or ambulatory/primary care setting) or comparable experience
• Maintain and enhance professional nursing skills and knowledge through continuing education

For Licensed Practical Nurses (LPN):

• Certificate of graduation from a Licensed Practical Nursing program at an accredited U.S. school of nursing
• Current and unrestricted Massachusetts LPN license
• Basic Life Support (BLS) Certification
• 1 year of professional nursing experience (ED, urgent care, or ambulatory/primary care setting) or comparable experience
• Maintain and enhance professional nursing skills and knowledge through continuing education

For Both RN and LPN:

• Excellent interpersonal, communication, and organizational skills
• Proficiency in computer skills including EPIC, Outlook, Microsoft Office, OneDrive, etc.
• Excellent written and verbal communication skills (bilingual helpful)
• Excellent judgment and ability to solve problems in a timely manner
• Ability to adhere to strict confidentiality standards
• Demonstrated ability to work effectively both as a team member and independently
• Demonstrated ability to work in a culturally diverse and inclusive environment

Pay

The salary range for this position is $32.00/hourly to $45.00/hourly. Actual compensation may vary based on factors such as experience, qualifications, and other job-related criteria.
