Responsibilities

• Deliver field-based care: conduct comprehensive primary care visits and physical exams in patients' homes
• Manage chronic disease: create longitudinal care plans for high-risk patients
• Collaborate across the continuum: partner with interdisciplinary team for seamless care transitions
• Prevent ER visits: treat acute needs in the home to keep seniors safe and out of the hospital

Requirements

• Master's Degree (NP) with preferred focus on Adult, Geriatric, Acute Care and/or Family
• Active, unencumbered NP license and Board Certification (ANCC or AANP) in Pennsylvania
• Valid driver's license and reliable vehicle for local field-based travel
• Must be legally authorized to work in the United States without employer sponsorship

Pay

Salary range: $104,763 - $166,750.00 per year, plus mileage reimbursement and full benefits. Relocation assistance available. $10,000 sign-on bonus.
