Nurse Practitioner - Traveler

About the role

We are seeking a Nurse Practitioner to provide home-based medical care across Colorado, focusing on managing chronic illnesses in elderly and complex patient populations.

Responsibilities

• Managing chronic conditions, preventing hospitalizations, and educating patients to improve care quality and satisfaction
• Documenting clinical encounters while meeting quality measures like HEDIS and recapturing HCCs
• Conducting comprehensive geriatric assessments covering functional, social, safety, cognitive, and mental health aspects
• Triaging, diagnosing, and treating patients in-home using various equipment and medications
• Ordering and interpreting tests such as x-rays, labs, and EKGs, and initiating treatment at the point of care
• Collaborating with patients' PCPs and specialists to coordinate care
• Implementing quality initiatives to enhance care management and delivery

Qualifications

• Certification through AANP or ANCC
• Unrestricted NP and RN licensure in Colorado
• At least 2 years of clinical experience in primary, geriatric, palliative, urgent, or home-based care
• Valid driver's license and reliable transportation for travel within Colorado

Preferred Skills

• Knowledge of CPT, ICD-10, HCPCS codes
• Understanding of clinical standards, NCQA, CMS, Medicaid/Medicare guidelines
