Nurse Practitioner - Travel

Seeking a Nurse Practitioner for home-based medical care across Colorado, with travel covering approximately 85% of the time. The role includes managing chronic illnesses in elderly patients, focusing on wellness, preventing hospitalizations, and improving care quality. Responsibilities include conducting comprehensive geriatric assessments, triaging and treating patients in their homes, ordering and interpreting tests, and collaborating with primary care providers and specialists. The position requires certification through AANP or ANCC, current unrestricted RN and NP licenses in Colorado, and at least 2 years of clinical experience in primary care, geriatrics, or urgent care.
