Responsibilities

  • Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Nurse Practitioner (NP).
  • Collaborates with a multidisciplinary healthcare team to ensure high-quality, evidence-based outcomes.
  • Develops and implements comprehensive care plans for patients.
  • Counsels and educates patients on various health topics.
  • Coordinates patient care across different departments and providers.
  • Participates in interdisciplinary team meetings to discuss patient cases and develop treatment plans.
  • Assesses patient needs and provides ongoing treatment based on clinical judgment and evidence-based practices.
  • Fosters a positive patient-provider relationship through effective communication and empathy.
  • Maintains accurate and up-to-date medical records for each patient.
  • Advises patients on lifestyle modifications and preventive measures.
  • Monitors patient progress and adjusts treatment plans as necessary.
  • Collaborates with other healthcare professionals to optimize resource utilization and improve patient outcomes.
  • Engages in professional development activities to enhance clinical skills and knowledge.
  • Participates in quality improvement initiatives to enhance patient care and satisfaction.

Requirements

  • Bachelor's degree in Nursing or related field from an accredited institution.
  • Current licensure as a Registered Nurse (RN) in the state of [State Name].
  • Advanced Practice Registered Nurse (APRN) certification from an approved organization.
  • At least 2 years of experience in a clinical setting, preferably in a healthcare provider setting.
  • Experience in providing direct patient care and managing patient populations.
  • Strong interpersonal and communication skills, including the ability to work effectively with diverse groups of people.
  • Ability to manage multiple tasks and prioritize responsibilities.
  • Proficiency in electronic health record systems and other relevant software tools.

Qualifications

  • Master's degree in Nursing or related field from an accredited institution.
  • Experience in advanced practice nursing roles such as NP, PA, or Clinical Nurse Specialist (CNS).
  • Experience in leading interdisciplinary teams and collaborating with physicians, social workers, and other healthcare professionals.
  • Experience in research and publication in peer-reviewed journals.
  • Experience in teaching and mentoring students or new nurses.
  • Experience in grant writing and securing funding for healthcare programs.
  • Experience in policy development and implementation in healthcare settings.
