Responsibilities
- Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Nurse Practitioner (NP).
- Collaborates with a multidisciplinary healthcare team to ensure high-quality, evidence-based outcomes.
- Develops and implements comprehensive care plans for patients.
- Counsels and educates patients on disease prevention, treatment options, and self-care practices.
- Coordinates patient care across various departments and providers.
- Assesses patient needs and collaborates with other healthcare professionals to develop and implement treatment plans.
- Maintains accurate and up-to-date medical records and documentation.
- Participates in quality improvement initiatives and patient safety programs.
- Continuously updates knowledge and skills through continuing education and professional development.
Requirements
- Current licensure as a Nurse Practitioner in the state where the position is located.
- At least 2 years of experience in a clinical setting, preferably in a healthcare environment.
- Strong interpersonal and communication skills, including the ability to work effectively with diverse populations.
- Excellent problem-solving and decision-making abilities.
- Proficiency in electronic health record systems.
- Ability to manage multiple tasks and prioritize responsibilities effectively.
- Passion for improving patient care and outcomes through innovative approaches.
Qualifications
- Master's degree in Nursing (MSN) or related field from an accredited institution.
- Active NP license in the state of practice.
- Experience in a clinical setting, preferably in a healthcare environment.
