About the role
Avir Health Group is searching for a qualified Nurse Practitioner (Skilled Nursing) to join our family. This position is focused in a Skilled Nursing Facility (SNF) setting, where you would serve as a clinical leader, working collaboratively with the facility's care team to oversee both short- and long-term residents.
Responsibilities
- Admissions, discharges, monthly stability checks, and evaluations for changes in condition
- Advanced care planning, comprehensive assessments, and prescribing treatments
Requirements
- Active Texas Advanced Practice Licensure (APN) and DEA License
- Previous nursing or clinical experience
- Strong communication and teamwork skills
- Master's degree in Nursing
