Responsibilities

  • Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing setting.
  • Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizations.
  • Deliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-being.
  • Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvement.
  • Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement efforts.
  • Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and discharges.
  • Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term health.
  • Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standards.

Requirements

  • Master's degree in Nursing from an accredited Nurse Practitioner program.
  • Active and unrestricted Advanced Practice Nurse (APN) license in respective state.
  • Current DEA registration and prescriptive authority (or eligibility).
  • Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
  • 2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings.
  • Strong clinical assessment, diagnostic, and treatment planning skills.
  • Knowledge of chronic disease management and geriatric care principles.
  • Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizations.
  • Proficiency in evidence-based practice and clinical decision-making.
  • Ability to work both independently and collaboratively within an interdisciplinary team.
  • Excellent communication and interpersonal skills with residents, families, and staff.
  • Strong organizational, time management, and documentation skills.
  • Familiarity with electronic health records (EHR) systems.
