About the role
Belle is seeking a compassionate and clinically skilled Nurse Practitioner (NP) to provide remote telehealth support for Medicare beneficiaries in our Chronic Care Management (CCM) program. This role focuses on responding to urgent clinical escalations, primarily involving poorly controlled hypertension, medication concerns, and other non-life-threatening acute issues. You will assess members remotely, determine appropriate care, and collaborate with nurses, Community Health Workers, primary care providers, and care coordinators to help keep members healthy and out of the emergency department when clinically appropriate. This is a fully remote position supporting a fast-growing, mission-driven organization.
Responsibilities
- Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns
- Evaluate symptoms, review medical histories, and determine appropriate interventions
- Determine when escalation to the primary care provider, emergency department, or 911 is clinically indicated
- Document all encounters accurately and timely in the electronic medical record
- Assist in reducing avoidable emergency department visits and hospitalizations through proactive clinical intervention
- Provide patient education on disease management, medications, lifestyle modifications, and symptom monitoring
- Promote medication adherence and self-management of chronic conditions
- Provide clinical guidance and education to nursing staff on appropriate escalation pathways
- Participate in clinical case reviews and quality improvement initiatives
Requirements
- Active, unrestricted Nurse Practitioner license (Georgia preferred)
- National certification appropriate to specialty
- Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management
- Experience managing hypertension and common chronic conditions, including diabetes, heart failure, COPD, and chronic kidney disease
- Prior telehealth or virtual care experience
- Strong clinical assessment and decision-making skills
- Excellent verbal communication and patient education abilities
- Familiarity with eClinicalWorks (eCW) or similar EMRs
