About the role

As an IKC Nurse Practitioner (NP), you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.

We provide comprehensive care and complex chronic condition management to patients nationwide, whether they are in the early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant, or receiving life-sustaining dialysis. Responsibilities

  • Complete comprehensive health assessments
  • Manage complex patients, focused on reducing hospitalizations and improved health outcomes
  • Cover PST & MST time zones
  • Accommodate patient and dialysis center availability with flexibility

Requirements

  • Passion for delivering care to patients with complex, chronic illnesses
  • Minimum of two (2) years' experience as a Nurse Practitioner (NP)
  • Current unrestricted NP license to practice in the state
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook
  • Home office and high-speed internet
  • Willingness to obtain up to 6 additional state licensures for remote management of patient population

Preferred Qualifications

  • Experience working in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage (MA) risk perspective and conducting Health Risk Assessments on high-risk/chronically ill patients
  • 5+ years’ experience as a Nurse Practitioner practicing in the adult/gerontology populations
  • Spanish-speaking preferred in some markets
  • Experience with longitudinal management of chronic illness (CHF, DM, CV disease, HTN)

Schedule

  • Monday–Friday, full-time, self-managed schedule
  • No nights, weekends, or on-call

Pay

The salary range for this role is $118,000.00 – $135,000.00 per year. Compensation depends on qualifications, skills, competencies, and experience. Location-specific minimum wage details are as follows:

  • New York Exempt: New York City and Long Island: $66,300.00/year; Nassau, Suffolk, and Westchester counties: $66,300.00/year; remainder of New York state: $62,353.20/year
  • New York Non-exempt: New York City and Long Island: $17.00/hour; Nassau, Suffolk, and Westchester counties: $17.00/hour; remainder of New York state: $16.00/hour
  • Washington Exempt: $80,168.40/year
  • Washington Non-exempt: Bellingham: $19.13/hour; Burien: $21.71/hour; Everett: $20.77/hour; Unincorporated King County: $20.82/hour; Renton: $21.57/hour; Seattle: $21.30/hour; Tukwila: $21.65/hour; remainder of Washington state: $17.13/hour
