Responsibilities

The Nurse Practitioner delivers annual risk assessments in residential settings or via telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.

• Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions, and determine physical, mental, and social needs.
• Take patient history, perform examinations, and determine diagnoses.
• Provide written documentation of patient visits per NCQA standards.
• Take patient vital signs as necessary.
• Place case management referrals and communicate with PCPs as needed.
• Communicate with patients, caregivers, agency nurses, providers, and vendors to ensure proper diagnosis.
• Perform clinical duties while observing OSHA Universal Precautions.
• Maintain patient confidentiality.
• Attend required meetings, in-services, and participate in committees as requested.
• Participate in professional development activities and maintain professional licenses and affiliations.

Qualifications

Required

• Active/unrestricted nurse practitioner license in coverage states.
• Board certification from ANCC, AANP, or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid driver’s license and good driving record.
• Outstanding EHR skills.

Preferred

• Geriatric training/experience.
• Skill in teamwork and maintaining effective working relationships.

Conditions of the Role

• Adaptability to varying weather conditions and patient home environments.
• Full range of body motion, including handling/lifting patients.
• Manual and finger dexterity, eye-hand coordination, normal visual acuity, and hearing.
• Standing, bending, walking, and stair climbing.
• Regular lifting/carrying of items up to 50 pounds.
• Ability to ride in a vehicle up to 150 miles daily in urban and/or rural settings.
• Ability to drive, if necessary.
