About the role

We are offering a $1,500 Sign-On Bonus for a Nurse Practitioner role focused on in-home health assessments.

Responsibilities

• Conduct comprehensive assessments in members’ homes or facilities, reviewing health status, medical history, medications, and social factors to improve care and identify undiagnosed conditions.
• Perform adult/geriatric assessments, documentation, and coordinate with healthcare teams.
• Administer testing and collaborate with physicians and caregivers.
• Work flexible hours, including evenings and weekends, with travel up to 80-90%.

Qualifications

• A Master’s degree or equivalent
• Valid NP license
• Board certification
• 1+ year experience
• Willingness to travel
• Valid driver’s license
• Strong communication skills
