Role Overview
Provide comprehensive in-home health assessments to older adults and at-risk individuals, reviewing medical history, medications, social factors, and health risks during visits of up to one hour. Collaborate with healthcare teams, conduct assessments, order tests, and document findings to improve care quality and close health gaps. Services are delivered at members' homes or facilities.
Key Responsibilities
- Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, and education
- Administer and order point-of-care and lab tests
- Collaborate with physicians, case managers, social workers, and caregivers
- Coordinate with primary care providers for follow-up and education
- Travel to members' residences as needed
Qualifications & Skills
- Master's Degree or equivalent and NP licensure with prescriptive authority
- Board certification in relevant specialty areas
- Current BLS, ACLS, or CPR certification
- At least 1 year of NP experience
- Ability to travel extensively, with valid driver's license and insurance
- Strong computer, communication, and documentation skills
