About the Role
Provide comprehensive in-home health assessments for members, reviewing their medical history, current health, medication adherence, social environment, and risks during visits of up to one hour. Collaborate with healthcare professionals and family members to improve care quality, identify undiagnosed conditions, and support care management.
Responsibilities
- Conduct adult and geriatric assessments, documenting according to protocols
- Administer and order point-of-care and lab testing as needed
- Coordinate with physicians, case managers, social workers, and caregivers
- Educate patients and follow up in collaboration with primary care providers
- Perform assessments at members’ homes or facilities
Requirements
- Master’s Degree or equivalent experience with NP licensure
- Current state licensure with prescriptive authority; board certification in relevant specialty
- CPR/BLS/ACLS certification
- Minimum one year of NP experience
- Ability to travel extensively, including overnight and weekend travel
- Valid driver’s license and auto insurance
- Strong computer skills and excellent communication abilities
