Nurse Practitioner (PRN) - In-Home Health Assessments

Role Overview

Seeking a Nurse Practitioner (NP) to conduct comprehensive health assessments for members in their homes or facilities. You will evaluate health status, medical history, medication adherence, and social factors during visits up to one hour, helping to identify care gaps and improve outcomes.

Key Responsibilities

• Perform adult/geriatric assessments, including history, diagnosis, treatment, and health education
• Administer and order point-of-care and lab testing
• Collaborate with physicians, case managers, social workers, and caregivers
• Coordinate with primary care providers for follow-up and patient education
• Work in members' residences or skilled nursing facilities

Qualifications & Skills

• Master's Degree or equivalent, NP licensure, and prescriptive authority
• Board certification in relevant specialty (e.g., Adult, Family, Gerontology)
• Current BLS/ACLS or CPR certification
• At least 1 year of NP experience
• Ability to travel extensively, with valid driver's license and insurance
• Strong communication, computer skills, and familiarity with medical records software
