Nurse Practitioner (PRN) - In-Home Health Assessments*

About the role

This position is based in Russell, PA and requires regular travel to the area. Candidates must have or be willing to obtain a Pennsylvania NP license. Matrix Medical Network pioneered the first national in‑home clinical network and provides comprehensive health assessments to older adults and at‑risk individuals, delivering personalized care that improves health outcomes and lowers costs.

Responsibilities

• During visits lasting up to one hour, review and observe a member’s current health, medical history, medication adherence, social environment and other risks.
• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up and clear documentation per Matrix guidelines.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers and ancillary medical personnel as appropriate.
• Collaborate with the primary care physician on patient education and follow‑up.
• Provide services at the member’s place of residence (home or skilled nursing facility).

Qualifications

• Master’s degree or commensurate experience with satisfactory completion of NP licensure.
• Current NP licensure in the state of practice, including prescription authority or ability to obtain it.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
