Role Overview
Responsibilities
- Conduct comprehensive health assessments in members' homes or care facilities
- Review health history, medications, social factors, and risks during visits up to one hour
- Collaborate with healthcare teams to improve care quality and address care gaps
- Administer tests as necessary
- Provide patient education and follow-up
Qualifications & Skills
- Master's Degree or equivalent with NP licensure and prescriptive authority
- Board certification in adult, gerontology, family, emergency, or acute care
- Current BLS, ACLS, or CPR certification
- At least 1 year of NP experience
- Ability to travel extensively, with a valid driver’s license and insurance
- Strong communication skills and familiarity with medical records software
