Responsibilities
- Conduct comprehensive in-home health assessments for older adults and at-risk individuals
- Review medical history, medication adherence, social factors, and risks during visits lasting up to an hour
- Conduct adult/geriatric assessments
- Administer tests
- Collaborate with healthcare professionals
- Provide follow-up care at members' homes or facilities
Qualifications
- A master's degree or equivalent experience
- Current licensure with prescriptive authority
- Certification in adult or family primary care
- BLS/CPR certification
- At least one year of NP experience
- Ability to travel extensively
- A valid driver’s license
- Strong communication skills
