About the role
Join a team providing in-home health assessments to improve patient outcomes and manage care gaps.
Responsibilities
- Conduct comprehensive assessments of adult and geriatric patients, including medical history, diagnosis, treatment, and social factors.
- Administer and interpret point-of-care and lab tests.
- Collaborate with physicians, case managers, social workers, and caregivers to develop and follow up on care plans.
- Provide education and referrals as needed, documenting all interactions per protocols.
- Perform services at members' residences, including homes and Skilled Nursing Facilities.
Qualifications and Skills
- Master’s Degree or equivalent, with NP licensure and prescriptive authority in practice state.
- Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care specialties.
- Current BLS, ACLS, or CPR certification.
- Strong communication, computer skills, and a valid driver’s license with insurance.
- Experience preferred but new graduates encouraged to apply.
