Role Overview: This position involves conducting comprehensive in-home health assessments for adult and geriatric patients, focusing on medical history, medication adherence, social factors, and health risks. The provider collaborates with healthcare teams, including physicians, case managers, and family members, to ensure personalized care and identify health issues. Services are delivered at patients' residences, including homes and skilled nursing facilities.
