About the role

Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise. Responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education.

Responsibilities

  • Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, family history, psychosocial situation, cultural factors, and performing appropriate physical examination.
  • Order/perform appropriate laboratory diagnostic and other screening tests. Seek additional information as needed, including consultation with physicians and other clinicians, for evaluation of illness.
  • Integrate data to determine diagnosis and therapeutic plan, including identification of any health risks.
  • Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Educate patients regarding health promotion/illness prevention and recommend community resources.
  • Prepare documentation for EMR including updating patient records with examination and test results, diagnosis, medications, and treatment.
  • Participate in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities.
  • Act as consultant/collaborator to clinical team on area of specialty. Comply with patient confidentiality requirements and promote patient advocacy.
  • Collaborate with physicians in managing acute and long-term medical needs of patients. Provide monitoring and continuity of care between visits, including triaging patient calls/emails.
  • Elicit and document a complete health history, perform and document physical examinations, and order appropriate diagnostic tests.
  • Assess patient’s perception of health status, personal strengths, support systems, and identify patients/families at risk for healthcare problems.
  • Formulate diagnoses based on health history, physical examination, and diagnostic tests. Develop and document healthcare plans with patients and families.
  • Establish, review, and document short and long-term goals in collaboration with patients.
  • Demonstrate ability to provide appropriate care based on individual needs, including age-specific care.
  • Institute treatment plans based on assessments and interpretations of diagnostic tests. Counsel patients according to motivation and compliance.
  • Provide telephone advice and triage for acute and unscheduled patients. Document all pertinent data and interventions in patient records.
  • Provide refills for routine medications and take appropriate action when reviewing abnormal lab data or diagnostic tests.
  • Consult with other providers as indicated.
  • Evaluate patient understanding and compliance with healthcare plans through regular follow-up and reassessment.
  • Re-evaluate short and long-term goals with patients and participating health professionals.
  • Identify learning needs of patients and family members. Provide individualized health education and develop teaching goals.
  • Function as a clinical resource person for staff.
  • Coordinate patient care activities and health care planning among multiple providers, patients, and families.
  • Communicate and consult with family, staff, and unit personnel prior to and during hospitalization. Participate in discharge planning when appropriate.
  • Promote ongoing communication with community agencies to facilitate continuity of patient care.
  • Promote patient and family participation in planning care. Assist patients in obtaining information for informed decision-making and support their decisions.
  • Inform patients of the Patient Bill of Rights and provide referrals to community resources when appropriate.

Qualifications

Nurse Practitioner:

  • BSN required; MSN preferred.
  • Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program.
  • Current Registered Nurse license as defined by the California Board of Registered Nursing.
  • Active and unrestricted Nurse Practitioner license in California.
  • Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable).
  • Must hold current national board certification by first day of work and maintain it.
  • Must hold an active and transferable DEA license to California as required by clinical specialty.
  • Must be eligible for payer enrollment with CMS Medicare and Medicaid.
  • BLS and ACLS Certification (American Heart Association) required.
  • 2 years of experience in advanced practice or specialty preferred.
  • 2 years in specialty area highly desired.
  • Prior experience in a medical office environment preferred.

Pay

Pay Range: $87.58 - $102.18 - $119.77 per hour.

Schedule

  • 8-hour day shifts.
  • Scheduled weekly hours: 40.
