About the Role

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to residents in assisted living communities throughout the Port Jervis and Middletown, NY area. This is an excellent opportunity for a provider who enjoys building meaningful relationships with seniors while maintaining a healthy work-life balance. You'll work alongside an interdisciplinary healthcare team to deliver comprehensive, patient-centered care focused on improving quality of life for older adults in a part-time, outpatient primary care setting.

Responsibilities

  • Perform comprehensive health assessments and physical examinations.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop individualized treatment and care plans.
  • Conduct medication reconciliation and medication management.
  • Order, interpret, and review laboratory and diagnostic tests.
  • Prescribe medications and treatments within the scope of practice.
  • Collaborate with physicians, nursing staff, and interdisciplinary care teams.
  • Educate patients and families regarding disease management, preventive care, and healthy lifestyle practices.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Participate in quality improvement initiatives and clinical care planning.
  • Ensure compliance with all clinical, regulatory, and documentation standards.

Qualifications

  • Master's Degree in Nursing from an accredited Nurse Practitioner program.
  • Active New York State Nurse Practitioner license.
  • Active DEA registration.
  • Currently enrolled and credentialed with both Medicare and Medicaid.
  • Minimum of 3 years of clinical experience in primary care or related practice.
  • Ability to practice independently in accordance with New York State regulations.
  • Strong clinical assessment, diagnostic, and patient management skills.
  • Excellent communication and interpersonal skills.

Preferred Qualifications

  • Experience caring for geriatric patients.
  • Experience in assisted living, skilled nursing, or long-term care settings.
  • Experience with electronic medical records (e.g., eClinicalWorks).

Schedule

Monday-Friday, 30 hours per week with a flexible schedule and no nights or weekends.
