Nurse Practitioner-Pre Heart Transplant - Mount Sinai Hospital - Full Time - Day

Full-time, day shift position at Mount Sinai Hospital for a Nurse Practitioner specializing in pre-heart transplant care.

About the Role

A Nurse Practitioner (NP), as defined by Title VIII, Article 139 section 6910 of the New York Education Law, is a Registered Nurse (RN) with separate certification through advanced clinical nursing education in a distinct specialty. NPs are independent practitioners who diagnose, treat, and prescribe for patients within their specialty. Medical orders, progress notes, and treatment plans do not require countersignature by a physician.

Responsibilities

Clinical Practice

• Assess the health status of patients and families through comprehensive health history and physical examination.
• Initiate and order therapeutic and diagnostic procedures, consultations, and laboratory examinations.
• Prescribe medication regimens, interpret findings, and alter the plan of care according to accepted standards and practice guidelines.
• Educate and counsel on health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.
• Document findings and plan of care in the patient’s medical record.
• Consult with physicians according to established standards, policies, procedures, and protocols.
• Demonstrate knowledge and skills necessary to provide care based on physical, psychosocial, educational, safety, and age-related criteria.
• Initiate referrals to specialty services and foster continuity of care.
• Participate in team meetings and conferences to enhance interdisciplinary care delivery.
• Maintain patient and employee confidentiality.
• Develop and record the plan of care based on diagnoses and individual patient needs, including interpreting client responses.
• Perform patient record reviews with the collaborating physician at least every three months if using a collaborative practice agreement.
• Complete Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) as required for credentialing.
• Practice as a member of the allied health staff according to Medical Staff rules and hospital bylaws.

Requirements

• Document competency in specialty-specific procedures as part of orientation and annual performance review.

• Work within a Collaborative Practice Agreement or Collaborative Practice Relationship Attestation.
• Physical stamina and manual dexterity for a variety of tasks.

Qualifications

Education

• Master’s degree in a program designed to prepare an NP to diagnose, treat, and prescribe within their specialty, or completion of an approved post-master’s NP certificate program.
• New York State RN license with current registration.
• Certification by the New York State Department of Education as a Nurse Practitioner with current registration:
  • Graduate of a New York State registered NP program or equivalent, or certified as an NP by a national association via examination.
  • Completion of a pharmacy component (minimum required semester hours) and instruction in New York State and Federal laws relating to prescriptions and recordkeeping (3-hour course).
  • Collaborative Practice Agreement with a physician and designated protocols if less than 3,600 hours of NP experience, or Collaborative Relationship with a physician if employed at Mount Sinai Hospital with over 3,600 practice hours.

Experience and Certifications

• Progressive experience relevant to the clinical practice area.
• National Board Certification in Advanced Practice Nursing required for all NPs hired after June 1, 2019, or for those who bill for professional services, regardless of hire date (may be deferred up to 1 year with approval).
• Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges.
• Drug Enforcement Administration (DEA) certificate.
• Basic Life Support (BLS) certification.
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), or Neonatal Resuscitation Program (NRP) certificates as required by specialty.
