About the Role
We are seeking a full-time Nurse Practitioner or Physician Associate to provide compassionate outpatient care across all ages.
Responsibilities
- Conduct comprehensive exams, diagnose, treat, and follow up with patients.
- Make referrals to internal and community partners as needed.
- Deliver acute and preventive care in an outpatient setting.
- Document patient encounters accurately in electronic health records.
- Communicate effectively with patients, families, and healthcare staff.
- Develop and update clinical protocols.
- Participate in team meetings and support quality improvement initiatives.
- Maintain licensure, certifications, and continuing education requirements.
- Prescribe medications in accordance with established protocols.
- Deliver culturally competent, patient-centered care.
Requirements
- Master’s degree in Nursing or related field.
- Current Nurse Practitioner license in New Hampshire.
- 1–5 years of experience in primary care, preferably with underserved populations.
- American Heart Association BLS certification (or willingness to obtain).
