About the Role

We are seeking a full-time Nurse Practitioner or Physician Associate to provide compassionate outpatient care across all ages.

Responsibilities

  • Conduct comprehensive exams, diagnose, treat, and follow up with patients.
  • Make referrals to internal and community partners as needed.
  • Deliver acute and preventive care in an outpatient setting.
  • Document patient encounters accurately in electronic health records.
  • Communicate effectively with patients, families, and healthcare staff.
  • Develop and update clinical protocols.
  • Participate in team meetings and support quality improvement initiatives.
  • Maintain licensure, certifications, and continuing education requirements.
  • Prescribe medications in accordance with established protocols.
  • Deliver culturally competent, patient-centered care.

Requirements

  • Master’s degree in Nursing or related field.
  • Current Nurse Practitioner license in New Hampshire.
  • 1–5 years of experience in primary care, preferably with underserved populations.
  • American Heart Association BLS certification (or willingness to obtain).
