Nurse Practitioner / Physician Assistant - Pulmonary - SNF

Responsibilities

• Conduct pulmonary assessments for patients in post-acute care settings
• Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure
• Develop and implement individualized treatment plans
• Collaborate with interdisciplinary teams
• Educate patients, families, and facility staff on respiratory care and treatment plans
• Maintain accurate and timely EMR documentation
• Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events

Qualifications

• Nursing: 3 years (Required)
• PA or NP: 1 year (Required)
• License/Certification: BLS/BCLS (Preferred)
• Experience: Preferred a minimum of 6 months of experience in post-acute care setting, Preferred experience in Pulmonary
• Education: Preferred a minimum of 6 months of experience in post-acute care setting, Preferred experience in Pulmonary
• Master’s Degree in Nursing from an accredited Nurse Practitioner program or Master’s Degree from an accredited Physician Assistant program required
• CPR BCLS/BLS certified
• Strong communication, organizational, and teamwork skills
• Ability to work independently in the clinical environment
