Using an evidence-based approach focused on quality care management and data-driven medical decisions, Bluestone care teams manage patients’ chronic conditions, address social determinants of health, manage transitions to and from inpatient settings, provide behavioral health support and more.

Under our model of care, Bluestone patients experience 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period. Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

About the role

  • Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
  • Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
  • Manage a panel of 200 patients (approximately 12-16 patients per day with 14 being the average)
  • Ability to design and manage your schedule
  • Develop and implement care plans appropriate to patients and their desires
  • Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
  • Access to diagnostics such as labs and x-rays as needed
  • Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
  • Board Certified/Board Prepared through AANP, ANCC, or NCCPA
  • Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
  • Ability to obtain and/or maintain DEA certification for the State of required work
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications

  • 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
