Responsibilities
- Conduct comprehensive primary care visits for geriatric patients in Skilled Nursing Facilities (SNFs) and other post-acute care settings.
- Diagnose, treat, and manage acute and chronic medical conditions while promoting preventive care and improving overall patient outcomes.
- Evaluate changes in patient condition and develop individualized treatment plans.
- Review and reconcile medications to ensure safe and effective therapy while minimizing polypharmacy risks.
- Order, interpret, and follow up on laboratory tests and diagnostic studies.
- Care for patients across multiple facilities and coordinate care with facility staff, specialists, therapists, and family members.
- Accurately document patient encounters and maintain complete, timely medical records.
- Maintain independent clinical decision-making while collaborating closely with supervising physicians and the interdisciplinary care team.
Qualifications
- Board-certified Nurse Practitioner or Physician Assistant.
- Active Indiana license (or eligible for licensure).
- 3+ years of Nurse Practitioner or Physician Assistant experience preferred.
- Interest or experience in geriatrics, primary care, or post-acute medicine.
- Comfortable practicing independently in multiple skilled nursing facilities.
- Excellent communication and interpersonal skills.
- Organized, detail-oriented, and committed to providing exceptional patient care.
Pay
Salary: $120,000 annually
Productivity bonus up to $20,000 annually
