Position Summary

Collaborates with Physicians
- Works within approved scope of practice.
- Obtains complete medical history and physical data on patients.
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.
- Performs a complete physical exam and records findings.
- Collaborates with physicians in managing the medical needs of patients.
- Orders appropriate laboratory and diagnostic procedures.
- Educates and advises patients regarding health and illness prevention.
- Recommends community resources to meet patient and family needs.
- Supports patient access and care coordination through triage, follow-up, and management of urgent situations.
Supports BMG Programs
- Participates in health education.
- Maintains related records and statistical data and prepares statistical reports as needed.
- Participates in interdisciplinary care conferences and assists with referrals and dissemination of information to appropriate vendors.
- Assists in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
Organizational Responsibilities
- Attends and participates in department meetings and is accountable for all information shared.
- Completes mandatory education, annual competencies, and department-specific education within established timeframes.
- Completes annual employee health requirements within established timeframes.
- Maintains licensure/certification and registrations in good standing throughout the fiscal year.
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
- Adheres to regulatory agency requirements, survey process, and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.
Education/Licensure/Certification
- Graduate of an accredited Physician Assistant or Nurse Practitioner master’s program.
- Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date.
- BLS obtained within 1 Month (30 days) of hire date or job transfer date if required.
- American Heart Association or American Red Cross accepted.
- Current BLS and ACLS certifications if required.
Knowledge & Skills
- Thorough knowledge of practice standards, policies, and procedures of BMG and the appropriate professional organization.
- A high level of analytical skills necessary to assess the patient and participate in follow-up care planning.
- Interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals, and paraprofessionals.
- Effective communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
Working Environment and Physical Demands
- Working in a clinical environment requires physical exertion, frequent changes in job demands, and exposure to biohazards.
- Requires physical ability and stamina (i.e., to walk/stand for prolonged periods of time, push carts/wheelchairs, lift up to 50 pounds, to position/lift patients at a maximum of 35 pounds without assistance, provide CPR, etc.) to perform the essential functions of the position.
