Nurse Practitioner/Physician Assistant, Hospital Medicine
Job Summary
Cooley Dickinson Health Care, an affiliate of Mass General Brigham, is advancing health care in western Massachusetts. The Hospital Medicine Nurse Practitioners (NPs) and Physician Assistants (PAs) work collaboratively with physicians, consultants, nurses, case managers, and rehabilitation teams to deliver high-quality, efficient care to hospitalized patients.
Key Responsibilities
- Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders
- Maintain patient progress and coordinate care plans with attending physicians and interdisciplinary teams
- Document ongoing care through progress notes
- Manage discharges, including medication reconciliation and coordination of follow-up care to support safe transitions
- Educate patients and families on diagnoses, medications, procedures, and post-discharge care
- Triage and facilitate patient transfers from the Cooley Dickinson Hospital Emergency Department, outside hospitals, and outpatient settings
- Communicate with accepting providers to ensure seamless transitions of care
Qualifications
- Master's Degree
- Physician Assistant required
- No experience in lieu of a degree accepted
Additional Job Details
- Prior APP experience in inpatient medicine strongly preferred
- Prior experience with EPIC EMR preferred
Summary
The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation.
