Nurse Practitioner / Physician Assistant - Full-Time/Part-Time

About the role

Seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care in home settings within Northeast Oklahoma City. The provider will work under physician supervision to assess, diagnose, and develop treatment plans for patients with chronic conditions, primarily focusing on geriatric and hospice populations.

Responsibilities

• Provide high-quality, patient-centered care through physical exams, medical history reviews, and tailored care plans.
• Communicate effectively with staff and clinical teams via internal platforms.
• Order diagnostic tests, prescribe medications, and implement necessary interventions.
• Build trust with patients, families, and caregivers during visits.
• Develop impressions, diagnoses, and treatment strategies.

Qualifications & Skills

• Board-certified with valid NP or PA license in Oklahoma.
• Strong clinical assessment, diagnosis, and treatment skills.
• Positive attitude, excellent communication, and problem-solving abilities.
• 1-2 years of primary care experience with geriatric patients; hospice experience preferred.
