Responsibilities

  • Conduct comprehensive hospital-based assessments and create treatment plans for high-needs members
  • Collaborate with market MD to identify critically ill members
  • Perform in-home assessments including history, physical exams, and safety screenings
  • Engage in end-of-life care discussions as necessary
  • Collect laboratory specimens during home visits when appropriate
  • Identify diagnoses for active medical management focusing on chronic disease
  • Educate patients and families, emphasizing close monitoring and resource referrals

Qualifications

  • Active North Carolina nurse practitioner/physician assistant license
  • Board certification (NP: AANP/ANCC; PA: NCCPA) within six months of hire
  • At least one year prior clinical experience
  • Medicare Part D prescriber certification
  • Valid BLS certification (ACLS preferred)
  • Experience in geriatric care and home care preferred

Pay

$126,422 – $189,634 annually
