About the role

Access Medical Clinic (AMC) is seeking clinicians to join our team that have the desire, drive, and dedication to assist AMC in providing great care to patients of all ages. We are hiring a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality, patient-centered care across clinic and long-term care (LTC) settings. This position will provide coverage in both a clinic and LTC setting. You must have the ability to travel within the assigned territory.

Responsibilities

  • Provide comprehensive evaluations and ongoing management for clinic and LTC patients (acute, chronic, and post-acute)
  • Develop and adjust individualized care plans; prescribe medications and therapies per regulations
  • Order, interpret, and act on labs, imaging, and other diagnostic studies
  • Manage high-risk LTC conditions (e.g., infections/sepsis risk, wounds, CHF/COPD, dementia/behavioral symptoms, polypharmacy, end-of-life/goals-of-care conversations)
  • Long-Term Care focus
  • Care coordination with physicians, facility leadership, nursing, and interdisciplinary teams; support admissions, transitions of care, and resident/family communication
  • Stabilization of medically complex residents within the facility
  • Reduction of potentially avoidable hospital transfers and ED utilization
  • Support for regulatory readiness and clinical quality performance
  • Improvement of condition-change response, escalation pathways, and care coordination
  • Enhancement of communication with residents, families, and interdisciplinary teams
  • Support for regulatory compliance, documentation readiness, infection prevention, and quality improvement initiatives
  • Accurate and timely documentation of patient/resident encounters in the EHR/EMR; adherence to AMC protocols, HIPAA/OSHA, and infection-control standards
  • Practice with strong independent clinical judgment within scope; escalation to collaborating physicians/specialists as needed; prioritization of early intervention and resident safety
  • Effective use of EHR/clinical systems and leveraging of clinical data/telehealth tools as appropriate

Requirements

  • Graduate degree as NP or PA (MSN/DNP or equivalent; MPAS/DMSc or equivalent)
  • Active, unencumbered state license and current board certification (AANP/ANCC for NP; NCCPA for PA)
  • DEA registration; BLS (or ability to obtain); DOT certification (as required)
  • 2+ years clinical experience preferred; LTC/SNF/geriatrics strongly preferred
  • Strong clinical assessment/diagnostic skills, patient-centered communication, and EMR proficiency
