About the Role
In-person position at skilled nursing facilities. Part-time schedule available. The culture is detail-oriented, outcome-focused, stable, people-oriented, and team-oriented.
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communicate and collaborate with the patient's primary care team to ensure the best outcomes.
- Complete timely charting and documentation (can be done from home).
Requirements
- Licensed Nurse Practitioner (Required)
- Certified Nurse Practitioner (Required)
- Licensed Physician Assistant (Required)
- Certified Physician Assistant (Required)
- Must be licensed in TN
- Reliable transportation
