Nurse Practitioner/Physician Assistant

Total Extended Care Services, a growing Physical Medicine and Rehab group, is looking for a dedicated Nurse Practitioner or Physician Assistant to partner with our PM&R physician in the sub-acute rehab setting. This role is ideal for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.

About the Role

Location: In-person at skilled nursing facilities in Ohio.

Responsibilities

• Partner with the facility's therapy department to provide the best possible rehab care.
• Communicate and collaborate with the patient's primary care team to ensure optimal outcomes.
• Complete timely charting and documentation (can be done from home).
• Provide on-site pain management with a focus on reducing re-admission rates.
• Coordinate care with medical providers, therapists, social workers, nurses, and families.

Requirements

• Licensed Nurse Practitioner or Certified Nurse Practitioner (Required).
• Licensed Physician Assistant or Certified Physician Assistant (Required).
• Must be licensed in Ohio.
• Reliable transportation.
