About the Role

We seek compassionate Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high-quality in-home and telehealth care to patients with multiple chronic conditions such as diabetes, hypertension, chronic kidney disease, heart failure, COPD, and depression. This role involves working within an integrated multidisciplinary team to establish long-term patient relationships, provide continuity of care, and support disease management, preventive care, and medication education.

Responsibilities

Conduct patient assessments in home and telehealth settings
Provide patient education on disease management and preventive care
Coordinate care with multidisciplinary teams
Document patient encounters and care plans accurately
Participate in virtual team meetings

Requirements

Active license as a Nurse Practitioner or Physician Assistant
Board certification (NP: ANCC/AANP or PA: NCCPA)
Current DEA certificate or eligibility
Minimum of 2 years of patient care experience
Reliable transportation, valid driver’s license, and auto insurance

Skills

Critical thinking and independent decision-making
Knowledge of federal/state regulations (HIPAA, OSHA)
Experience with EHR systems
Preferred experience in managed care, population health, or comprehensive wellness visits
