About the role

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.

Responsibilities

  • Provides collaborative primary care and services to a panel of members
  • Delivers ISNP Model of Care activities, develops and implements advanced care planning, and executes a treat-in-place model to avoid unnecessary hospitalizations
  • Facilitates and administers Health Plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth
  • Serves as a trusted advisor to members, loved ones, and nursing home staff and colleagues
  • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and interdisciplinary care team members on members’ care needs, goals of care, treatment plans/changes
  • Supports overall market goals/targets and balances member caseloads
  • Promotes Longevity Health with members, loved ones, nursing facilities, and other stakeholders
  • Functions autonomously and is accountable for practice
  • Establishes meaningful relationships and effectively functions in the nursing home environment
  • Is member-centric and customer service-oriented, establishing meaningful relationships that ensure effective and high-quality care for Longevity Health members
  • Performs other related duties as assigned

Requirements

  • 6+ months experience as a Nurse Practitioner or Physician Assistant
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program
  • Experience working in the nursing home environment is strongly desired
  • Computer/Technology proficiency
  • Ability to engage and communicate effectively with physicians, members, and family members
  • Self-directed but functions well in a team environment
  • Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
  • Demonstrated critical thinking skills, organization, and prioritization skills

Qualifications

  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant, NCCPA board certified
  • DEA License required
  • Active national provider number (NPI) and an active Medicare provider number
  • Eligible to participate in the Medicare program as a provider and registered
  • Valid state driver's license with a good driving record and proof of automobile insurance
  • Auto liability insurance declaration page with minimum limits required by your state of residence (proof required annually)
