Responsibilities
- Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
- Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
- Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
- Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
Requirements
- Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
- State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
- Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
Pay
$62.22 - $84.18 per hour plus mileage reimbursement.
