Responsibilities

• Deliver field-based care: conduct comprehensive primary care visits and physical exams in the comfort of a patient's home.
• Manage chronic disease: create longitudinal care plans that move the needle for high-risk patients.
• Collaborate across the continuum: partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER visits: treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Requirements

• Advanced degree: Master's Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
• State required credentials: active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable mobility: valid driver's license and vehicle for local field-based travel.

Pay

$62.22 – $84.18 per hour + mileage reimbursement.
