Responsibilities

• Assess patients’ medical and mental health needs and develop proactive care plans that manage conditions and address barriers to care.
• Ensure compliance with local, state, and federal agencies related to clinical services provided.
• Prescribe medications and order lab work, diagnostic procedures, and consultations.
• Monitor patients’ compliance and response to treatment, modifying plans as needed.
• Provide training and advice to facility staff as required.

Requirements

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3+ years’ experience in clinical nursing, rehabilitation, or geriatric populations, and/or Fee for Service experience.
• Experience with Electronic Health Records (EHR).
• Working knowledge of Microsoft applications, including Word, Outlook, and Excel.
